  1. In case of an emergency, call 911 or go to the nearest Emergency Room.

  2. You have 3 days from the application form submission date to submit written cancellation of your membership without penalty and receive a full refund. Refund will be processed within 5 business days.

  3. Submit written requests for membership cancellation at least 2 business days before the end of the month. You will be billed 100% of the membership fee if canceled late or during the 3-month commitment.

  4. Written cancellation or hold requests can be submitted in person or by email to CaylorForge@gmail.com.

  5. You will be billed $50 for the application fee.

  6. You will be billed $200 (Full membership) or $100 (Part-time membership) recurring using cash or Credit Card (Visa, MasterCard, or Discover) on the 1st of each month. A membership commitment is required, followed by month-to-month membership.

    • The membership, $600 paid in a one-time payment or multiple payments monthly, is required, regardless of attendance.

    • If the payment fails to process, membership benefits will cease until payment is made

    • Missed facility use/instruction time due to poor attendance/non-payment will not be credited/prorated.

  7. You will be billed late fees for membership dues paid after 11:59 pm on the 1st day of the month.

    • A $15 late fee will be assessed if payment is made between the 2nd and 5th of the month

    • An additional $10 fee will be assessed to reactivate your account if paid after 11:59 pm on the 5th of the month

  8. Non-payment will subject the membership to termination if the member:

    • Fails to remit payment before 11:59 pm on the 5th day of the month

    • Accrues more than 2 non-consecutive late payments in a 12-month period

  9. Failure to make timely payments results in membership termination and the account will be sent to collections. Re-instatement will be withheld for a minimum of 18 months or at management’s discretion.

  10. Reimbursement/prorated fees will not be provided for any reason.

  11. Membership may be placed on hold up to 2 months per year. Hold requests are required in writing at least 2 days before the end of the month to prevent the payment from processing.

    • If the request is submitted by the 5th day of the month, received payment will be credited to a future month.

    • If the member cancels the membership before the credit is used, the credit is non-refundable.

  12. A parent/guardian must be present on the premises. A minor may be accompanied and released to people with photo identification listed in the minor's profile as an "additional parent/guardian" or an "emergency contact" at the request of the parent/guardian. If the parent/guardian or listed delegate leaves the minor unattended on the premises:

    • The minor will immediately be removed from class and authorities will be contacted if applicable.
